How Do I Schedule A Physical Therapy Appointment For The First Time?

If this is your first Physical Therapy visit, you will need to schedule an Initial Evaluation BY CALLING US. Follow up Physical Therapy apppointments can be scheduled on-line at your convenience, with our staff when you make your Initial Evaluation, or during your treatments. It is recommended that you make them as soon as possible to get the times that are good for you.

1. SELF PAYMENT options for Physical Therapy. This gives the Therapist and the Patient complete control when it comes to all aspects of treatment. There are no added costs for dry needling.
2. INSURANCE PAYMENT for Physical Therapy: Limited by insurance companies in terms of duration or how long the treatment can be provided. Often, there needs to be a referral for therapy provided by your Doctor. Medicare and replacements always require a referral from your MD. INSURANCE DOES NOT COVER massage, craniosacral, dry needling therapy, or classes offered. Insurance also does not cover the cost of supplies such as theraband ($5.00), dry needling supplies ($25.00 for 6 treatments, non-refundable), iontophoresis supplies ($10.00 per pad).

What Is Your Cancellation Policy?

Please give us 24 hours cancellation notice so that we can allow other patients who need care to access the open times. You are able to cancel your appointment with a call to our office or you can do this through the text that you receive when you are scheduled. If you cancel with less than 24 hours notice, and we are unable to fill the time slot that you held, there is a $70.00 fee.

What Else Does Bodylogic Offer?

We offer Integrative Massage Therapy, Craniosacral Therapy and Dry Needling Therapy. In the near future, we will be offering BALANCE TRAINING CLASSES, and PILATES REHAB Therapy.

Do I Get Appointment Reminders?

This appointment software will send you a text and/or e-mail of your appointment time when you schedule, AND 24 hours before your appointment. If you need to cancel, please do so prior to the 24-hour notification or there will be a fee billed to you.

Am I Able To See All Of My Scheduled Appointments?

SCHEDULE APPOINTMENTS or choose MY APPOINTMENTS. Under the ‘my appointments’ button you can see the full list of the appointments that you have. They will ask for your mobile phone number, they will text you a code and you simply enter that code for all of your appointments.

What Do I Need To Do Before My First Appointment

On our website www.BodylogicNC.com, you can go to the Patient Center to print out our forms. Please complete prior to your Initial Evaluation. This makes the intake more efficient so we can spend more time with you. If not, we can do that in the office. Please bring the following: A. Insurance cards if you choose to have us file. B. Drivers License. C. prescription for therapy if applicable–always with Medicare plans. D. clothing that you can move around in comfortably.
